Over-the-Scope Clip for Control of a Recurrent Diverticular Bleed

چکیده
An 87-year-old man with a history of diverticular bleed and abdominal aortic stenting presented with 3 episodes of hematochezia. Colonoscopy identified multiple diverticula, including one that was actively bleeding in the sigmoid colon (Figure 1). Two resolution clips were placed in the bleeding diverticulum with hemostasis (Figure 2). Twelve hours later, the patient had recurrent hematochezia. The previously placed aortic stent prevented cannulation of the inferior mesenteric artery by interventional radiology, so a repeat partial colonoscopy was performed without preparation, revealing fresh blood surrounding the 2 previously placed clips. An upper endoscope (Olympus 190 HQ; Olympus, Center Valley, PA) was fitted with a 13.6mm over-the-scope clip (OTSC®; Ovesco, Tübingen, Germany) that was deployed over the diverticulum and the previously placed clips (Figure 3). The bleeding stopped immediately. The patient did not require surgery and was discharged the next day; he was followed over period of 4 months after discharge and had no further bleeding.

Use of an over-the-scope clip and a colonoscope for complete hemostasis of a duodenal diverticular bleed.
Duodenal diverticular bleeding has traditionally required invasive surgical repair. With improvements in endoscopic devices, endoscopic treatment has become the main therapy for such bleeding [1,2]. However, the question of which endoscopic devices should be used is still controversial because of complications and recurrent bleeding after treatment [3]. A newly developed device, the overthe-sco...
متن کاملAssociation between the Risk Factors for Atherosclerosis and Diverticular Bleeding
Background and purpose: Diverticulosis is a major health problem in middle-aged and older adults. The cause of bleeding in the diverticulum is not clear, but arteriolar changes are considered as the main pathogenesis of diverticular bleeding. This study aimed at investigating the relationship between the risk factors for atherosclerosis and diverticular bleeding. Materials and methods: In this...
متن کاملTreatment of acute colonic diverticular bleeding in high risk patients, using an over-the-scope clip: a case series.
The prevalence of diverticulosis has increased in industrialized countries in recent years and the condition is reported in up to 60% of patients older than 70 years [1,2]. Diverticular disease is one of the most common causes of lower gastrointestinal bleeding and accounts for 30%– 40% of all such bleeds [3,4]. Many diverticular bleeding episodes resolve spontaneously. However, according to th...

uation of melena and hematochezia. The initial esophagogastroduodenoscopy, a colonoscopy, and a computed tomo− graphic scan all failed to identify the source of the bleeding. Double−balloon enteroscopy was performed using the oral approach (EN−450T5/20; Fujinon Co. Ltd., Saitama, Japan). A blood clot adher− ing to a jejunal diverticulum was found 20 cm distal to the ligament of Treitz and linea...
متن کاملClosure of a Chronic, Non-Healing Tracheoesophageal Fistula With a New Over-the-Scope Clip
Tracheoesophageal fistulas can arise from both benign and malignant conditions, and often present clinically as increased secretions, coughing with or without hypoxia after swallowing, and recurrent aspiration pneumonia. We describe a tracheoesophageal fistula in a post-chemotherapy lymphoma patient that was closed with use of an over-the-scope Aponos clip.
